Registration Information

Registration fees include all educational
sessions and CME processing, full breakfasts, breaks, lunch, and
reception. Registration is limited and will be taken on a
first-come, first-served basis. Participants will receive a
written confirmation of registration after it has been processed
by the AAPM&R office.
PASSOR reserves the right to limit registration on the
basis of experience level.

Before You Register… A Reminder:

Registration is limited and will be
accepted on a first-come, first-served basis. Attendees are
encouraged to register early. No on-site registrations will
be accepted.

Each registrant must complete a
separate registration form for each course. Photocopies are
acceptable for additional registrations. Retain a photocopy of
each form for your records.

Faxed registration forms are accepted.

Participants will receive a written confirmation of their
registration after it has been processed.

Nonrefundable airline fares are the
responsibility of potential registrants, not AAPM&R/PASSOR.

Full payment in U.S. funds must accompany
the registration form. Registration forms received without
payment will not be processed and could result in a loss of
opportunity to attend the course requested. Checks should be
made payable to PASSOR.

additional time). Full payment in U.S. funds must
accompany the registration form.
Participants may register for both February and September
2008 courses. However SP 101 and SP 201 are taught concurrently. Participants
may only register for one course level per workshop. Please note: A separate registration form must be completed and
submitted for each workshop.
Cancellations and Refunds
Requests for cancellation of meeting registration must be
made in writing to the AAPM&R national office. Cancelled registration is subject
to a $150 processing fee. No refund requests will be honored for cancellations
postmarked later than January 25, 2008 for the spring workshop, May 9, 2008 for
the summer workshop, and
August 22, 2008 for the fall workshop, or for "no-shows."
Nonrefundable airline
