Now Available: 2025 MIPS Exception Applications

CMS has released its MIPS Exception Applications for the 2025 performance period, which are open through December 31, 2025. Clinicians may apply for one of two available exceptions to indicate their inability to report data for certain MIPS performance categories due to circumstances outside their control:

Extreme and Uncontrollable Circumstances (EUC) Exception Application: The MIPS EUC Exception allows MIPS-eligible clinicians, groups, and virtual groups to request reweighting of any or all MIPS performance categories when impacted by circumstances beyond their control. These circumstances are defined as rare events that prevent the collection or submission of performance data, significantly disrupt normal processes, or affect the ability to meet MIPS requirements for an extended period (e.g., unable to collect quality data for three months).

Promoting Interoperability (PI) Hardship Exception Application: The PI Hardship Exception is available for clinicians who meet specific hardship criteria (e.g., insufficient internet connectivity, decertified EHR technology, etc.) and is limited to the Promoting Interoperability performance category.
