The Centers for Medicare & Medicaid Services (CMS) has released 2025 Merit-based Incentive Payment System (MIPS) performance feedback and final scores. It is important that physiatrists who were eligible to participate in MIPS in 2025 review this information since final scores will directly impact Medicare Part B payment adjustments in 2027.
AAPM&R members are encouraged to review their performance feedback carefully and verify that all submitted data, scoring calculations, and payment determinations are accurate. For PM&R physicians and rehabilitation practices, MIPS performance can have a meaningful impact on future Medicare reimbursement.
Targeted Review Period Now Open
With the release of final scores, CMS has opened the 2025 MIPS Targeted Review process. Clinicians and group practices that believe an error occurred in the calculation of their final score or payment adjustment may request a review from CMS. The targeted review period is expected to remain open for approximately 60 days and will close 30 days after CMS releases 2027 MIPS payment adjustments, which are anticipated within the next month.
Importantly, targeted review determinations are final and are not eligible for further CMS appeal or review.
Situations That May Warrant a Targeted Review
PM&R physicians and practices should consider requesting a targeted review if they identify errors such as:
- Data submitted under an incorrect Taxpayer Identification Number (TIN) or National Provider Identifier (NPI).
- Qualifying APM Participant (QP) status not being recognized, resulting in an inappropriate MIPS final score and payment adjustment.
- Qualifying for performance category re-weighting due to a special status designation, Promoting Interoperability Hardship Exception, or Extreme and Uncontrollable Circumstances Exception that was incorrectly applied.
- Suspected inaccuracies in category scores, final score calculations, or payment adjustment determinations.
If CMS approves a targeted review request and determines a scoring error occurred, the agency will update the final score and associated payment adjustment as soon as technically feasible. Supporting documentation may be required depending on the nature of the review request.
How to Access Your MIPS Feedback
To access 2025 MIPS performance feedback and submit a targeted review request:
- Sign in to the Quality Payment Program (QPP) portal using your HCQIS Access Roles and Profile (HARP) credentials:
https://qpp.cms.gov/login - Review your final score, payment adjustment information, and supplemental reports.
- Select "Targeted Review" from the left-hand navigation menu if you need to request a review.

Recommended Action for AAPM&R Members
AAPM&R recommends that physiatrists and rehabilitation practices:
- Review their 2025 MIPS performance feedback immediately.
- Verify quality, cost, improvement activity, and promoting interoperability data.
- Confirm that any applicable reweighting policies were properly applied.
- Monitor CMS announcements regarding 2027 payment adjustments.
- Submit a targeted review promptly if discrepancies are identified.
Because 2025 MIPS performance will directly affect Medicare reimbursement in 2027, physicians should take advantage of this opportunity to verify that their scores accurately reflect their reported performance and participation.
