The Centers for Medicare & Medicaid Services (CMS) recently released preliminary 2025 Merit-based Incentive Payment System (MIPS) final scores, as well as associated payment adjustment amounts that will impact Medicare Part B payments in 2027.
Physicians and practices are encouraged to log in to their secure account on the Quality Payment Program (QPP) website to review their final score and payment adjustment information. A final score above 75 points will receive a positive payment adjustment, a score of 75 points will receive a neutral adjustment, and a score below 75 points will receive a negative adjustment.
Physicians and organizations that believe their score or payment adjustment was calculated incorrectly may, for a limited time, request a targeted review.
Important Deadline
Requests for a targeted review must be submitted to CMS by November 30, 2026, at 8:00 p.m. ET.
Examples of circumstances that may warrant a targeted review include:
- Data submitted under the wrong Tax Identification Number (TIN) or National Provider Identifier (NPI)
- Incorrect application of Alternative Payment Model (APM) participation status
- Failure to apply eligible category reweighting due to extreme and uncontrollable circumstances
